Student Registration

Welcome to Knowlton Elementary!
We are ready to help you complete the
registration process.

STEP#1
Print the following forms, fill in the information and then bring the completed forms to the school office.

1.  Student Information Form
2. Guardianship Form
3.  Proof of Residency Form
4.  Special Services Form
5,  McKinney-Vento Form
6.  Kindergarten Registration Information (Kindergarten Only)
7.  Media Opt Out Form
8.  School Fee Notice (if applicable)
9. School Fee Waiver (if applicable)

STEP #2
Visit the office to complete registration.

Required documents to bring to the Knowlton Office
- Original Birth Certificate (we will make a copy)
- Original Immunization Records (we understand you may not have all
  completed, but please bring the current records)
   - Immunization Requirements
- Guardian Photo ID
- Two proofs of residency

When registering your student for kindergarten, you will be asked to request half-day or full-day Kindergarten. House Bill 517 has passed and requires schools to provide a half-day class when 18+ students register for half-day kindergarten. In the case where less than 18 students register for a half-day, an option within a full-day class will be provided upon parental request.

Utah State Board of Education has been tasked by the legislature to establish the minimum standards of half-day kindergarten. This process will involve collecting various stakeholder feedback and working through the State Board's process. We anticipate this determination to be finalized in the Board's June meeting.

We will communicate information as we know to registered incoming Kindergarten parents, as well as posting it on our website.
